1.        Fill out the Application Form for Disbursing the First Payment Compensation .

2.        The automatic calculation papers, which includes:

2.1           Subscription statement.

2.2           The compensation allocation decision.

2.3           The expense approval.

2.4           The decision notification.

3.        Proof of the cancellation of the work permit and the Ministry of Labor’s stamp of approval of the disbursement (with the exception of Palestinians and residents of the Gaza Strip).

4.        A photocopy of any of the following personal identification cards:

4.1           Foreign passport.

4.2           Jordanian passport that does not have a Jordanian national number.

5.        A letter from the Civil Status Department stating that the insured person does not have a Jordanian national number (for Palestinians and residents of the Gaza Strip who do not have a Jordanian passport with a Jordanian national number).

6.        An aquittance letter issued by the Income Tax Department for those whose due amounts are (1000) dinars or more (with the exception of the insured persons who are working in the West Bank).

7.        Disbursement voucher.

8.        The objections of the insured person and the departments concerned regarding the allocation decision.

9.        The decision of the Rights Settlement Committee on the reference form.

10.    Notification letter of the Rights Settlement Committee’s decision.

11.    The decision of the Social Security Affairs Committee.

12.    The Social Security Affairs Committee’s notification form.

13.    The decision of the Higher Court of Justice.

14.    An implementation memorandum from the Legal Department.

15.    Powers of attorney:

15.1       A photocopy of the authorized person’s identification card.

15.1.1      The personal identification card for the Jordanian individual.

15.1.2      The passport for the non-Jordanian individual.
